The cause of the air crash has never been revealed; the only information available to the public comes from four official reports. The investigation concluded that the crew, aircraft, airport operations, and air traffic control all met regulatory standards, with no technical faults, hazardous materials, or adverse weather reported at the time of the incident.
To study and commemorate the accident, Crater Surveyor reconstructed the crash site—a crater—using 612 paper-made wreckage pieces derived from the contents of these four reports.
